so that they may process your prescription and
apply the proper discounts. |

service and there are no forms or receipts to
submit. |

drugs. The Formulary PDF can be a helpful guide
to locating your Drug name and classification.
Generic drugs are not listed, as there are many.
If you would like to know what tier your drug
classifies as, or what alternatives may be available,
you may also contact your pharmacy or the Caremark
number located on your membership card. |

effective alternatives to higher priced
drugs of the same category and clinical
effectiveness. A generic drug may also be
